ToDo: Include Form Title [4]

Currently, only the template name is included in the ToDo list description. This forces the user to click the item to see what it is.

• [ ] A title is needed as well. Maybe tie it to the Title/Text input flag of "Primary Identifier"
• [ ] Form number would be nice as well.

I believe the Recent Updates area of the home screen is already doing this, so I'm hopeful we mirror that. Example here:

Here's an example of a ToDo list that illustrates that you can't decipher which submittal is which and some users have 15+ on this list.
