Currently, only the template name is included in the ToDo list description. This forces the user to click the item to see what it is.

<li>[ ] A title is needed as well. Maybe tie it to the Title/Text input flag of "Primary Identifier"</li>
<li>[ ] Form number would be nice as well.</li>

I believe the Recent Updates area of the home screen is already doing this, so I'm hopeful we mirror that. Example here:
