QR Code: Default Values

The default values added to the QR codes are not working. At QR creation for Rough Terrain Forklift on the Sandbox, I am adding Default Values for SDS equipment flag = True and SDS Equpment ID = any one of the options. However when scanning the qr code those fields are not filled out (but should be).
