PDF Export: Staple Attachments

When exporting a form, the attachments should be included as PDF's at the end of the document. This would be similar to how we export pictures in the Daily Report tool. Currently when exporting the form comes out in a pdf and any attachment are a hyperlink that opens separately forcing someone creating a complete document to do the stapling themself.
