Documents: Version info at Upload

I'm updating our Docs to include all the 466 stuff (and loving it!). I did realize we do need to give the user the opportunity to enter version/revision information when they initially upload a Drawing or Spec. for the first time. Otherwise anything from that first upload (many pages) will not contain that data and it will have to be added one page at a time.
