How are we logging Distribution?

<li>In the example below I distributed a form to the three people shown. However it looks like it was sent out multiple times because there are two entries.</li>
<li>The Distributed To is blank on one of the entries making it seem like it was distributed twice.</li>

Summary: Can we combine this one single distribution to show who it was sent to AND the file (what was sent) s one entry?
