Distribution History Enhancement

How are we logging Distribution?

1. In the example below I distributed a form to the three people shown. However it looks like it was sent out multiple times because there are two entries.
2. The Distributed To is blank on one of the entries making it seem like it was distributed twice.

Summary: Can we combine this one single distribution to show who it was sent to AND the file (what was sent) s one entry?
